Each paddler is required to fill out the Healani Membership Form and the HCRA Waiver Form
before the paddler gets in the practice boat. Please give the completed forms to your coach. 1. Healani Membership Form 2. HCRA Waiver Form New paddlers need to provide a copy of their Hawaii drivers license or Hawaii State ID. If you do not have an ID, provide a copy of your birth certificate. |
